In this vital role, you will support patients with daily activities, monitor health status, and promote independence while ensuring safety and comfort. Your expertise will help improve quality of life for those receiving home health services, including seniors, individuals with disabilities, and those managing chronic or acute conditions. This position offers an opportunity to make a meaningful difference in the lives of your patients through personalized, respectful care. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately Support medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations and medical documentation standards Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to ensure a safe and comfortable environment Observe patient condition closely, noting changes in mental status or physical health, especially in cases involving dementia or Alzheimer's care Assist with specialized care needs including catheter care, hospice support, memory care, and working with individuals with developmental disabilities or disabilities Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate documentation of patient interactions and medical records Provide compassionate companionship and emotional support to foster a positive environment for patients in assisted living or home settings Experience Prior experience working as a Home Health Aide or in long-term care settings such as nursing homes or assisted living facilities Proven ability to work with individuals with disabilities, dementia, Alzheimer's disease, or other cognitive impairments Familiarity with medical terminology and procedures related to patient care and documentation Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are preferred Experience operating EMR/EHR systems for accurate record-keeping and compliance with healthcare regulations Strong patient observation skills combined with a caring attitude to recognize subtle changes in patient condition Ability to work independently in home environments while maintaining high standards of safety and professionalism Join us if you're passionate about providing top-quality home health care that empowers individuals to live safely and comfortably at home.
