Job Description
Overview We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This role involves assisting individuals with daily activities, ensuring their safety, and supporting their overall well-being. Ideal candidates will have a strong background in patient care, experience working with seniors or individuals with disabilities, and a genuine desire to improve quality of life through personalized caregiving. The position offers opportunities for professional growth within the home health and community care sectors. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' conditions, reporting any changes or concerns promptly Administer medications following prescribed schedules and protocols Perform medical tasks such as taking vital signs, catheter care, and assisting with medical equipment like Hoyer lifts Support patients with Alzheimer's, dementia, memory care, hospice needs, and other specialized care requirements Maintain accurate medical documentation using EMR/EHR systems while adhering to HIPAA regulations Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions Perform light housekeeping duties including cleaning and laundry to maintain a safe and sanitary environment Provide emotional support and companionship to promote mental well-being Document patient progress and communicate effectively with healthcare teams Experience Candidates should possess prior experience in home health or long-term care environments, including assisted living facilities or nursing homes. Experience working with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer's care is highly valued. Familiarity with medical terminology, medication administration, medical records management, and the use of EMR/EHR systems is preferred. Certifications such as CPR are required or highly recommended. Candidates with skills in hospice care, hospice support, working with people with disabilities, or using assistive devices like Hoyer lifts will have an advantage. A background in caregiving for seniors or individuals needing memory care enhances suitability for this role. Prior experience in home & community care settings that involve patient observation, meal preparation, light cleaning, and medication management is essential for success in this position.
