Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and independence. Monitor and observe patients' health status by taking vital signs such as blood pressure, pulse, respiration, and temperature. Support medication administration following prescribed protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies. Operate assistive devices such as Hoyer lifts and perform catheter care to ensure safety and hygiene. Assist with mobility using equipment like Hoyer lifts or transfer belts, ensuring safe patient handling at all times. Support clients with meal preparation, light housekeeping including cleaning and laundry, and maintaining a safe home environment. Document all patient interactions accurately while adhering to HIPAA regulations to protect client privacy. Collaborate with healthcare teams to coordinate hospice care or services for individuals with disabilities or chronic conditions. Skills Strong patient observation skills to detect changes in health status promptly. Experience working in assisted living facilities, nursing homes, or home health environments. Familiarity with EMR/EHR systems for medical documentation and record keeping. Knowledge of medical terminology relevant to home health care practices. Certification in CPR and first aid to respond effectively during emergencies. Ability to perform catheter care, medication administration, and vital signs measurement accurately. Compassionate caregiver with experience working with seniors, individuals with developmental disabilities, dementia, or hospice needs. Excellent communication skills to interact empathetically with clients and their families. Ability to handle physical tasks such as lifting patients using Hoyer lifts safely and efficiently.
