This role offers an exciting opportunity to provide essential care in various settings, including assisted living facilities, group homes, and private residences. You will be a vital part of creating a safe, respectful, and nurturing environment that promotes independence and enhances quality of life. Whether assisting with personal hygiene, meal preparation, or medication management, your efforts will empower those you serve to live their best lives every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patients' health by taking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Use specialized equipment like Hoyer lifts or resident's lifts safely to assist with mobility and prevent injury. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Collaborate with healthcare teams to develop and implement personalized care plans that address each individual's unique needs. Skills Proven experience in home health, assisted living, nursing homes, or group homes with a strong understanding of patient observation and monitoring. Knowledge of Alzheimer's care, dementia care, memory care, autism support, and hospice care practices. Certifications such as First Aid, CPR, HIPAA compliance training, and medication administration are highly preferred. Ability to lift heavy objects safely and perform physically demanding tasks while maintaining patient safety. Excellent communication skills for working effectively with individuals with disabilities or developmental disabilities. Familiarity with working within care plans and documenting patient progress accurately.
