Job Description

Job Summary We are seeking compassionate and dedicated Personal Care Assistants to provide exceptional support and care to individuals in various settings, including home, community, assisted living. The ideal candidate will assist with daily living activities, monitor patient health, and promote independence while ensuring safety and comfort. This role offers an opportunity to make a meaningful difference in the lives of those with diverse needs, including seniors, individuals with physical disabilities. A strong commitment to patient dignity and well-being is essential. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Provide monitor vital signs in accordance with care plans Support patients with Alzheimer's, dementia, or memory care needs through specialized techniques Prepare nutritious meals and assist with meal planning and preparation Perform light housekeeping tasks including cleaning and maintaining a safe environment Utilize equipment such as Hoyer lifts or resident lifts to ensure safe transfers and mobility assistance Observe and document patient conditions, behaviors, and progress accurately in compliance with HIPAA regulations Follow prescribed care plans developed by healthcare professionals Provide emotional support and companionship to promote mental well-being Assist with medical tasks such as vital signs monitoring, medication reminders, and record keeping Work collaboratively with healthcare teams to ensure comprehensive patient care Skills Proven experience in home health, assisted living, preferred Knowledge of HIPAA regulations and patient confidentiality standards Certification in CPR and First Aid required; additional certifications such as Alzheimer's or Dementia Care are a plus Strong observational skills for patient monitoring and safety assessment Ability to work effectively with elderly Excellent communication skills for interacting with patients, families, and healthcare professionals Physical ability to perform heavy lifting when necessary and assist with transfers using equipment like Hoyer lifts Compassionate demeanor with a genuine desire to provide quality senior or disability care Ability to follow detailed care plans accurately and adapt to changing needs of patients This position is ideal for caring individuals committed to improving the quality of life for others through attentive caregiving.
