Days Location:
Travel to patients' homes. Currently covering 6 counties•Bartow, Floyd, Gordon, Catoosa, Whitfield, Murray. Uses the social work process to address the physical, emotional, psychosocial, and spiritual needs of patients and families across adolescent, adult, and geriatric populations. Reviews social and emotional factors affecting medical treatment and communicates with physicians and other team members for direction and clarification. Conducts systematic assessments of the physical, psychosocial, and functional aspects of patients and families to develop individualized plans with specific goals and outcomes. Identifes resources and community agencies to assist patients in accessing appropriate levels of care, providing short-term crisis intervention, and individual or family counseling when needed. Implements and evaluates the efficacy of care plans, making necessary changes and keeping physicians informed of updates. Documents thoroughly and submits records in a timely manner. Maintains up-to-date knowledge of community resources and referral processes. Other duties as assigned.
Knowledge, Skills, and Abilities:
Must possess some home visit experience. Demonstrates knowledge of community resources. Good listening skills and ability to acknowledge patient's preferences in an accepting manner. Ability to function independently, problem-solve, prioritize and make decisions related to patient care. Effective interpersonal skills in relating to referral sources, co-workers, and patients. Ability to communicate courteously with physician, clinical staff, and patients. Knowledge and competency to perform required documentation and patient care procedures as a MSW and to effectively manage various demands of the job. Demonstrates ability to communicate effectively in English, both verbally and in writing. Additional languages preferred. Ability to use various tools, equipment, and work aides as necessary to perform job duties. Background check meets organizational guidelines. Demonstrates moderate computer skills.
