In this vital role, you will provide personalized home healthcare services, ensuring safety, comfort, and dignity for patients across various settings. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document vital signs, medication administration, and medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide specialized care for clients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques and behavioral management strategies. Support patients during end-of-life care with compassion, dignity, and adherence to hospice care protocols. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home health regulations. Maintain patient confidentiality and adhere to HIPAA guidelines while managing sensitive medical documentation. Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met promptly. Skills Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments. Strong knowledge of medical terminology, catheter care, CPR, first aid, and basic life support techniques. Familiarity with home health agency operations, hospice care practices, and home & community care standards. Ability to perform patient transfers safely using various methods such as Hoyer lifts or other assistive devices. Excellent patient observation skills to monitor health status changes or behavioral cues. Experience working with EMR/EHR systems for documentation and medical records management. Compassionate caregiver with excellent communication skills capable of building trusting relationships with clients and their families.
