Responsibilities Assist clients with activities of daily living including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary Administer medications accurately following prescribed schedules and protocols while maintaining detailed medical documentation Monitor vital signs and observe changes in health status, reporting any concerns promptly to healthcare professionals Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, or behavioral health challenges Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation Maintain compliance with HIPAA regulations and ensure confidentiality of all patient information within electronic health record (EHR) systems like Epic or Cerner Follow aseptic techniques and infection control procedures during all caregiving activities to ensure safety for clients and staff Skills Proven experience working with individuals with disabilities, developmental delays, or behavioral health needs Knowledge of medical terminology, anatomy, physiology, and patient care procedures including catheter care and vital signs monitoring Familiarity with EMR/EHR systems such as eClinicalWorks or Home & Community Care platforms for accurate documentation Ability to perform complex caregiving tasks such as medication administration, medical-surgical procedures, and use of assistive devices like Hoyer lifts or wheelchairs Experience in acute care settings including ICU, emergency medicine, trauma centers (Level I & II), or post-acute care environments such as nursing homes or assisted living facilities Strong understanding of infection control practices including aseptic technique and infectious disease care protocols Excellent communication skills for working effectively with patients, families, and healthcare teams in diverse settings including outpatient clinics or hospital medicine units This rewarding role is ideal for caregivers passionate about delivering exceptional patient service across various healthcare environments.
