Full Job Description Duties:
- Complete clinical notes daily in the chart.
- Collaborate with Caregivers, Direct Professional, and Support Coordinators.
- Be creative and assist the individuals with activities and schedule calendars for their ISP goals.
- Assist individuals with daily living activities, including personal hygiene, meal preparation, and medication administration.
- Provide emotional support and companionship to individuals.
- Implement individualized care plans and behavior support plans.
- Assist with community integration and participation in recreational activities.
- Document individual progress and report any concerns or changes in behavior to supervisors.
- Ensure the safety and well-being of individuals at all times.
- Collaborate with a multidisciplinary team to provide comprehensive care .
