Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide high-quality support and assistance to individuals in various care settings. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and independence. Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report any irregularities to healthcare professionals promptly. Support medication administration following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary. Provide patient observation and monitor behavioral changes, especially in individuals with dementia, Alzheimer's, or neurodevelopmental disorders. Assist with patient transfers using appropriate methods such as Hoyer lifts or manual handling techniques to ensure safety during mobility. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities, or seniors in settings such as group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient care procedures including medication administration, patient monitoring, and emergency response skills like CPR and first aid. Familiarity with care plans, HIPAA compliance, dementia care, memory care, hospice care, and Alzheimer's care practices. Ability to effectively communicate with clients, families, and healthcare team members to ensure coordinated care. Experience operating patient transfer equipment such as Hoyer lifts and understanding resident's (personal needs) requirements. Capable of heavy lifting and manual handling necessary for patient transfers and assisting with mobility tasks. Excellent organizational skills for documenting patient observations, updating care plans, and maintaining accurate records related to home & community care activities.
