In this vital role, you will provide personalized care that promotes comfort, dignity, and independence. Your energy and empathy will help create a supportive environment for individuals receiving hospice care, ensuring their needs are met with kindness and professionalism. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts and other assistive devices Monitor and document patient observations, including vital signs, medication administration, and changes in condition using Electronic Health Record (EHR) systems while maintaining HIPAA compliance Provide specialized care for patients with dementia, Alzheimer's disease, or other memory-related conditions, ensuring a safe and comforting environment Support patients with catheter care, wound management, and other medical procedures under the supervision of nursing staff Assist with meal preparation, feeding, laundry, cleaning, and maintaining a clean and safe living environment in accordance with home & community care standards Record detailed medical documentation and updates in medical records to ensure accurate communication among healthcare providers Collaborate with interdisciplinary teams to deliver holistic hospice care tailored to each patient's needs Skills Strong ability to observe and respond to patient needs with compassion and professionalism Experience working in assisted living facilities, nursing homes, or home health environments Proficiency with EMR (Electronic Medical Records) or EHR systems for accurate documentation Knowledge of medical terminology relevant to hospice and long-term care settings Certifications such as CPR (Cardiopulmonary Resuscitation) and training in hospice care practices are highly valued Skilled in assisting with ADLs including bathing, dressing, grooming, meal prep, and mobility support using equipment like Hoyer lifts Experience caring for individuals with disabilities or developmental challenges is a plus Ability to perform vital signs measurement accurately and recognize signs of deterioration or distress Familiarity with medication administration protocols within scope of practice Strong communication skills for working effectively with patients, families, and healthcare teams Ability to handle sensitive situations with empathy while maintaining confidentiality under HIPAA guidelines Join us in providing compassionate hospice care that truly makes a difference.
