In this vital role, you will empower clients to live as independently as possible while ensuring their safety, comfort, and well-being. Responsibilities Assist clients with activities of daily living (ADLs), including personal care, grooming, dressing, and mobility support Monitor patients' health by taking vital signs and observing for any changes in condition or behavior Administer medication following care plans and HIPAA regulations to ensure safety and compliance Support clients with meal preparation, feeding, and nutritional needs tailored to their dietary requirements Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility Maintain detailed documentation of care provided, patient progress, and incident reports in accordance with privacy standards Engage in patient observation to identify needs or concerns promptly and communicate effectively with healthcare teams Requirements Prior experience working with individuals with disabilities, developmental or intellectual disabilities, or neurodevelopmental disorders is highly preferred Knowledge of group home settings, assisted living facilities, nursing homes, or home health environments is advantageous Certification in CPR, First Aid, and medication administration is preferred; willingness to obtain necessary certifications upon hire MANDT Certification Preferred; willingness to obtain necessary certifications upon hire Ability to perform heavy lifting and manual handling safely; familiarity with patient transfer equipment such as Hoyer lifts is a plus Strong communication skills for effective client interaction and collaboration with team members Experience working with individuals with autism, dementia (including Alzheimer's care), memory care needs, or hospice care is desirable Commitment to maintaining HIPAA compliance and respecting client confidentiality at all times Join us in making a positive impact by providing caring support that enhances the quality of life for those we serve.
