Do you have a passion for helping others? Are you tired of performing the same duties day after day? Optimae LifeServices is hiring a full or part-time Day Habilitation Direct Support Professional for our Des Moines Community Integration program. Experience in social services or mental health is preferred, but we are willing to train the right individual. Dependability and attention to detail are a must. Optimae provides services in Central Iowa for individuals living with disabilities, mental illnesses, and other barriers. Our Day Habilitation program is a group setting for individuals to work on developing independent living and social skills while getting out in the community and participating in activities with peers.

Schedule:

Monday - Friday 8:30 AM - 4:30 PM What You'll Do Collaborate with the Day Habilitation Coordinator to plan activities and skill building opportunities including volunteer experiences, guest speakers, and community outings. Provide direct care and assist assigned individuals with accessing the community and increasing independence with communication, socialization and daily living skills Implement individual's care plans and provide ongoing feedback regarding additional or changing care plan needs to Day Habilitation Coordinator Assure timely and accurate documentation What You'll Need Must be at least 18 years old Must pass criminal background checks Must have a valid driver's license and be willing to provide transportation in company 15-passenger van Basic computer skills to complete required documentation and online trainings Good oral and written communication skills and have the ability to accurately document all observations, significant events, and services provided.

Education:
