This energetic position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, ensuring safety, and supporting overall well-being. You will work closely with clients, families, and healthcare professionals to deliver high-quality home health services that promote independence and comfort. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions, including vital signs and overall health status Support patients with memory care needs, including Alzheimer's and dementia care, ensuring safety and comfort Document patient information accurately while maintaining HIPAA compliance and confidentiality Assist with transfers using patient transfer methods such as Hoyer lifts or other mobility aids Provide companionship and emotional support to clients, especially those receiving hospice or end-of-life care Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred. Demonstrated experience working with geriatric patients, individuals with developmental disabilities, or those requiring dementia or Alzheimer's care Knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) or CPR certification required Familiarity with home health agency operations, home & community care services, and hospice care standards preferred Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or other patient transfer devices Experience working within home health regulations and maintaining detailed medical documentation is highly valued Strong communication skills for effective client interaction and collaboration with healthcare teams This role offers a rewarding opportunity to provide essential care that enhances quality of life for your clients.
