New Hire Sign-On Bonus Shift:
Monday- Friday | 8:00 AM

Responsibilities Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility Provide light housekeeping, meal preparation, and companionship Support medication administration and simple procedures under RN guidance Observe and report changes in patient condition or home environment to the RN Case Manager Maintain accurate documentation and ensure compliance with care plans and regulatory standards Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundariesQualifications Education:
Completion of a 120-hour Nursing Assistant course. High School Diploma/GED High School diploma or GED License(s)/Certification(s): Valid Driver's License (IA, IL, or WI)- Required upon hire Basic Life Support (BLS) Certification
- Maintain current certification through the American Heart Association (Required upon hire) Certified Nursing Assistant (CNA)
- State of Iowa (IADIA) or Illinois (IDPH) preferred Child Abuse Mandatory Reporter Training
- Iowa or Illinois, required within 90 days of hire Adult Dependent Abuse Mandatory Reporter Training
- Iowa or Illinois, required within 90 days of hire
