Job Description

Job Summary We are seeking a dynamic and compassionate Home Care Aide Supervisor (HCA) to lead and coordinate our home care services, ensuring the highest quality of care for our clients. In this vital role, you will oversee a team of dedicated home care aides, manage care plans, and ensure compliance with health regulations. Your leadership will foster a positive environment where seniors and individuals with disabilities receive personalized support that promotes independence, dignity, and well-being. Duties Conducts monthly wellbeing phone calls to all assigned participants, as well as quarterly home visitations to assigned participants. Develop, implement, and monitor individualized care plans tailored to each resident's needs, including Alzheimer's, dementia, memory care, and disability services Conducts Introductory home visits for all newly assigned participants to assess the participant's home environment, introduce the assigned Home Care Aide and to provide key program and service delivery information. During the introductory home visit, the Supervisor also completes a comprehensive psychosocial and safety assessment. Prepares and reviews participants' Plans of Care (as directed by IDOA/CCU or MCO units) and instructs the assigned HCA regarding their assigned schedule and the services to be provided. Helps manage the intake process, including assignment of Home Care Aides and timely service initiation for new participants (e.g. within 15 calendar days for regular referrals and within 48 hours for referrals deemed interim status). Provides quarterly supervsion to assigned staff and document all quarterly meetings Ensures that all required probationary and annual performance evaluations are completed for assigned Home Care Aides according to established timelines. Requirements Proven experience in healthcare administration or senior living facility management with a focus on long-term care or assisted living environments Extensive knowledge of health regulation policies related to Illinos Department on Aging (IDOA) Strong leadership skills with supervisory experience managing multidisciplinary teams in healthcare or residential care settings Familiarity with dementia care, Alzheimer's support strategies, and developing effective care plans for residents with complex needs Proficiency in Microsoft Office applications for documentation and communication purposes Experience working with individuals with developmental disabilities or in disability services is highly desirable Join us as a Home Care Aide Supervisor to lead a dedicated team committed to delivering outstanding personalized care. $19.75 - $22.00 per hour
