Key Responsibilities:
Support assigned consumers with routine daily care to promote independence in accordance with their recovery plans. Use written instructions and modeling techniques to facilitate the development of independent living skills. Familiarize yourself with specific direct care interventions outlined in treatment plans. Assist consumers with activities of daily living, including personal hygiene, mobility, and medication management. Monitor and document consumer progress, reporting any concerns or changes to the supervising team. Maintain a clean, safe, and supportive environment for all consumers. Collaborate with multidisciplinary team members to ensure comprehensive care delivery.
Skills and Qualifications:
High school diploma or equivalent; additional training in healthcare or rehabilitation services preferred. Compassionate, patient, and respectful attitude towards consumers. Strong communication and interpersonal skills. Ability to follow detailed instructions and adapt to individual needs. Basic knowledge of safety procedures and infection control practices. Prior experience in a healthcare, rehabilitation, or social services setting is a plus. Ability to work effectively both independently and as part of a team.
