Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems. Support medication administration following prescribed instructions while adhering to home healthcare regulations and safety protocols. Provide specialized care for clients with conditions such as dementia, Alzheimer's disease, or developmental disabilities, including memory care and end-of-life support. Safely transfer patients using patient transfer methods like Hoyer lifts or other assistive devices, ensuring proper body mechanics and patient safety. Maintain patient confidentiality in compliance with HIPAA regulations while documenting medical records and care activities. Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and healthy environment. Requirements Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings. Certification in First Aid and Basic Life Support (BLS), including CPR training. Familiarity with home health agency operations and home & community care standards. Ability to work independently while effectively communicating with clients and healthcare teams. Physical stamina for heavy lifting and assisting patients with mobility challenges using appropriate patient transfer methods. Experience working with individuals with disabilities or geriatrics is preferred. Strong understanding of home health care assistance practices, including hospice care when applicable.
