Job Description

Job Summary Join our compassionate team as a Personal Care Assistant, where your dedication will make a meaningful difference in the lives of individuals requiring support with daily activities and personal well-being. In this vital role, you will provide personalized care, assist with daily living tasks, and promote independence in a safe, respectful, and nurturing environment. Your enthusiasm and commitment will help enhance quality of life for residents across various care settings, including assisted living, nursing homes, group homes, and home health environments. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to ensure comfort and dignity. Support residents with medication administration and monitor vital signs to maintain health and safety. Provide companionship and emotional support to foster a positive environment and reduce feelings of isolation. Prepare nutritious meals in accordance with care plans and assist residents during meal times to promote proper nutrition. Conduct patient observation and monitor for changes in condition or behavior, reporting any concerns promptly to healthcare professionals. Maintain cleanliness of resident rooms and common areas through regular cleaning duties to ensure a hygienic environment. Use specialized equipment such as Hoyer lifts or wheelchairs safely to assist with resident transfers and mobility needs. Follow all safety protocols including CPR, First Aid, HIPAA regulations for patient privacy, and infection control procedures. Collaborate with healthcare teams to develop and implement individualized care plans tailored to each resident's needs. Support residents' participation in social activities and community outings to promote engagement and well-being. Skills Proven experience in home & community care or long-term care environments; group home or nursing home experience highly preferred. Strong knowledge of assisting with ADLs, medication administration, vital signs monitoring, and patient observation techniques. Familiarity with Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or autism. Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or wheelchairs as needed. Excellent communication skills for effective interaction with residents, families, and healthcare professionals. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Ability to follow detailed care plans accurately while maintaining flexibility for changing resident needs. Basic cooking skills for meal preparation aligned with dietary requirements; experience in assisted living or home health settings is advantageous.
