Essential Responsibilities Provide personal care and assistance with activities of daily living, including bathing, grooming, dressing, toileting, and hygiene. Assist patients with mobility, positioning, and comfort measures. Provide companionship and emotional support to patients and families. Observe and report changes in the patient's condition to the appropriate hospice team member. Follow the patient's individualized plan of care. Maintain patient privacy, dignity, and confidentiality at all times. Assist with light housekeeping tasks related to the patient's immediate care. Maintain accurate and timely documentation of services provided. Communicate professionally and compassionately with patients, families, caregivers, and the interdisciplinary hospice team. Demonstrate dependability and a strong commitment to patient-centered care. Qualifications Current Home Health Aide certification, as required by applicable regulations. Previous healthcare, home health, hospice, or caregiving experience preferred. Compassionate and respectful approach to patient care. Reliable transportation and ability to travel to patient homes.
