Job Description

Overview Join our compassionate team as a Certified Nursing Assistant (CNA) or Direct Support Professional, where your dedication will make a meaningful difference in the lives of individuals requiring personalized care. This vital role involves providing hands-on support, ensuring safety, and promoting independence for residents across various care settings. Whether working in assisted living, nursing homes, or home and community environments, you will be a key contributor to enhancing quality of life through attentive, respectful, and professional care. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patient observations by tracking vital signs, assessing mental status, and reporting any changes promptly to healthcare teams. Administer medications and treatments following medical documentation protocols while adhering to HIPAA privacy standards. Support individuals with developmental disabilities, dementia, Alzheimer's disease, or hospice needs through specialized memory care and compassionate engagement. Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems accurately for documentation of patient care activities. Utilize medical terminology proficiently to communicate effectively with healthcare professionals and document patient information clearly. Use assistive devices such as Hoyer lifts and catheters safely during patient transfers and care procedures. Maintain a clean environment by assisting with laundry, cleaning duties, meal preparation, and ensuring infection control standards are met. Support patients' emotional well-being through active listening, companionship, and personalized care plans tailored to individual needs. Follow all safety protocols including CPR certification requirements and emergency response procedures for hospice or critical situations. Experience Prior experience working in nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated ability to provide patient-centered care with a focus on dementia, Alzheimer's care, memory care, or hospice settings. Knowledge of medical terminology, medication administration procedures, and vital signs monitoring is essential. Experience working with individuals with developmental disabilities or disabilities requiring long-term care enhances your effectiveness in this role. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping is advantageous. Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information. Ability to work independently or as part of a team in live-in employment or shift-based schedules within home & community settings.
