Salary Range:
$18.00 To $25.58 Hourly Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants. We are currently seeking a Health Aide to join our team to provide in center and home care services. Role:
The Health Aide provides personal care and supportive services to geriatric participants of the Elder Service Plan (ESP) PACE Program in both the PACE Adult Day Health Center and in participants' homes. Under the supervision of the Homecare Coordination Supervisor or Site Director, the PACE Aide delivers high-quality, person-centered care that supports participants' physical, emotional, and social wellbeing across settings. This is a dual-role position requiring flexibility, independent judgment, and the ability to reliably travel between the center and participant homes throughout the assigned workday.
Responsibilities:
Center Based Duties:
Provides personal care assistance including ADLs, hygiene, dressing, toileting, and mobility support. Assists with meals, feeding, and hydration monitoring. Escorts and assists participants with mobility needs, including wheelchair transport (up to 350 lbs.). Escorts participants to medical appointments within and outside of the center. Supports therapeutic and recreational programming as directed. Collaborates with nursing staff on participant assessments; identifies and communicates changes in condition. Assists in daily clinic operations and maintains a safe, clean participant environment. Attends interdisciplinary team morning huddles and care plan meetings. Receives and executes referrals for home-based services as assigned. Home 
Based Duties:
Receives and executes referral for home-based services as assigned. Provides personal care in the home including ADLs, skin care, nutrition support, light housekeeping, and laundry per the participant's plan of care. Adheres to participant schedules and individualized care plans. Communicates regularly with the Home Care Nurse and Homecare Nurse Manager regarding participant status. Builds trusting, professional relationships with participants and family members. Uses clinical observation skills to recognize and report changes in participants' physical or cognitive condition. General Duties Must be able to travel reliably between the PACE Center and participant homes as assigned during the workday. Completes all required trainings by due date and attends monthly staff meetings. Maintains regular and punctual attendance. Performs other related duties as assigned. Must be able to travel to and from multiple patients' homes and the work site in a timely manner.
Requirements:
High School Diploma or GED Home Health Aide Certification, Current Certified Nurse Assistant or Personal Care Assistant Certification required Current BLS (Basic Life Support for Healthcare Providers) Certification. One year of experience working with a Geriatric population is required, two or more years of experience with geriatric clients, preferably in a home setting preferred. Competency and command of the English language; must be able to read, write, speak and follow directions. Proven ability to to resolve problems creatively Demonstrated ability to follow directions; directions may be provided verbally or in written format Excellent organizational skills Excellent verbal and written communication skills Must possess think critically in order to solve problems recognize potential changes in the client from their usual state Ability to work independently and as part of a team Must be able to travel to and from site and patients homes in a timely manner and work occasional evenings, weekends or holidays Coursework in gerontology preferred All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or veteran status.
