Job Description
Job Summary We are seeking a compassionate and dedicated Home Health Aide (HHA) to provide exceptional in-home care to individuals with diverse needs. In this vital role, you will assist clients with daily activities, monitor their health status, and support their overall well-being. Your energetic approach and attention to detail will help promote independence and improve quality of life for those you serve. This position offers the opportunity to make a meaningful difference in the lives of patients while working in a dynamic, supportive environment. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and document patient vital signs including blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems Provide medication reminders and assist with medication administration following medical protocols and HIPAA regulations Support patients with memory care needs, including Alzheimer's disease and dementia care, ensuring a safe and comforting environment Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a hygienic living space Observe and report changes in patient condition or behavior promptly to healthcare professionals Assist with medical procedures such as catheter care, wound care, and hospice support as needed Qualifications Prior experience in home health care, nursing homes, assisted living or long-term care settings preferred Strong knowledge of medical terminology and patient care practices Certified in CPR (Cardiopulmonary Resuscitation) and able to demonstrate emergency response skills Experience working with individuals with developmental disabilities or disabilities in general is highly valued Ability to operate EMR (Electronic Medical Records) or EHR systems accurately for documentation purposes Knowledge of dementia, Alzheimer's, memory care techniques, and hospice care best practices Excellent communication skills; compassionate attitude; ability to work independently or as part of a team Valid driver's license and reliable transportation are often required for home visits Join us to be part of a dedicated team committed to delivering compassionate home health services that empower individuals to live safely and comfortably at home.
