As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care in a respectful and nurturing environment. Your energy, attention to detail, and genuine care will help improve the quality of life for those you serve. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs and reporting any changes to healthcare providers Administer medications and manage medical documentation accurately following HIPAA guidelines Support patients with memory care needs such as Alzheimer's disease and dementia through specialized care techniques Operate Electronic Health Record (EHR) systems to document patient progress and care activities precisely Use medical terminology correctly when recording observations or communicating with healthcare teams Safely utilize equipment such as Hoyer lifts or catheter care tools to assist with patient mobility and hygiene needs Provide companionship and emotional support, especially for individuals with developmental disabilities or those receiving hospice or long-term care Prepare nutritious meals and assist with light housekeeping tasks like cleaning and laundry to maintain a safe living environment Follow infection control protocols and maintain patient confidentiality in accordance with HIPAA regulations Qualifications Prior experience working in assisted living facilities, nursing homes, or home health environments preferred Knowledge of medical terminology, medication administration, vital signs monitoring, and medical documentation systems (EMR/EHR) Certification in CPR is required; additional certifications such as hospice care or dementia training are advantageous Experience working with individuals with disabilities or developmental challenges is highly valued Ability to lift safely using equipment like Hoyer lifts and perform tasks requiring physical stamina Strong communication skills for effective collaboration with healthcare teams and families Compassionate attitude with a commitment to delivering respectful, patient-centered care Must adhere to all safety standards including HIPAA compliance and infection control procedures Join us in providing exceptional home health services that empower individuals to live comfortably and confidently in their own homes.
