United States, Maryland, Greenbelt 6404 Ivy Lane (Show on map) Jun 26, 2026 About the Job General Summary of Position MedStar Health is seeking a Master's-level Social Worker (MSW) to join our Homecare team. In this role,you'llprovide critical social work services to both home health patients and clients under a Montgomery County contract.

You'lloffer counseling, connect patients with community resources, and collaborate with healthcare team members to address psychosocial factors affecting treatment, rehabilitation, and medical compliance. Key Responsibilities:
Provide psychosocial assessments and develop individualized care plans with the patient, family, and healthcare team. Deliver short-term counseling andfacilitateaccess to community resources as needed. Support patients with discharge planning, nursing home placements, and resolving social barriers to care. Maintaintimely,accuratedocumentation in compliance with federal/state regulations and agency policies. Participate in interdisciplinary meetings, committees, and community outreach on behalf of the department. Foster strong working relationships with internal teams and external partners. Stay current through ongoing education and training.
Qualifications:
Master's degree in Social Work(MSW) from a Council on Social Work Education (CSWE)-accredited program.
Active licensurerequiredupon hire:
LGSW, LCSW, LCSW-C, LISW, or LICSW. Minimum 2 years of experience in a healthcare setting. Valid driver's license and reliable personal vehiclerequired. Current CPR, BLS, and NRP certifications from the American Heart Association or American Red Cross. Home care experience preferred. This position has a hiring range of USD $61,838.00 - USD $111,259.00 /Yr.
