This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or long-term care. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions regularly, noting changes in health status or behavior Administer medications and perform basic medical tasks such as taking vital signs and catheter care under supervision or as trained Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality Support patients during transfers using proper patient transfer methods and equipment like Hoyer lifts or patient transfer belts Provide companionship and emotional support, fostering a positive environment for patients with dementia or memory care needs Follow home healthcare regulations and safety protocols during all caregiving activities, including first aid and basic life support (CPR) when necessary Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies preferred Demonstrated experience caring for geriatric patients, individuals with developmental disabilities, or those requiring hospice or end-of-life care Familiarity with medical terminology, medication administration procedures, and medical records management Hands-on experience using EMR/EHR systems for documentation and client communication Knowledge of home health care assistance standards and home & community care practices Experience working with individuals with disabilities or dementia-related conditions is highly valued Certification in first aid and CPR is required; additional certifications in hospice care or heavy lifting are advantageous Embark on a rewarding career where your dedication transforms lives!
