Job Description

Overview Join our dedicated team as a Direct Support Professional and become a vital part of empowering individuals with diverse needs to live fulfilling, independent lives. In this energetic role, you will provide compassionate, personalized care and support to residents in various settings, including group homes, assisted living facilities, and home care environments. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or resident's Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare teams Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient confidentiality Prepare nutritious meals and assist with meal planning and cooking to meet dietary needs and preferences Maintain a clean and safe environment through regular cleaning duties, laundry, and organization of resident spaces Support residents during outings or community activities to promote social engagement and independence Document care activities accurately in compliance with healthcare standards and facility policies Experience Prior experience working in group homes, assisted living, nursing homes, or home health care settings is highly preferred Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), autism support, hospice care, and working with individuals with disabilities or developmental challenges is advantageous Certification in CPR, First Aid, and basic nursing skills is desirable; ongoing training in HIPAA compliance and patient monitoring is a plus Experience with assisted living procedures such as medication administration, care planning, vital signs collection, and heavy lifting demonstrates readiness for this role Familiarity with home & community care practices including social work principles enhances your ability to deliver comprehensive support This role is ideal for compassionate individuals eager to make a positive impact through direct support work.
