In this vital role, you will provide personalized in-home care to clients, supporting their daily needs and promoting their independence. Your expertise will help improve quality of life for individuals requiring assistance with daily activities, while respecting their cultural preferences and traditions. This position offers an opportunity to make a meaningful difference in the lives of those you serve, fostering trust and comfort through culturally sensitive care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs and recognizing changes in condition Administer medications accurately following medical instructions and documentation protocols Support clients with memory care needs, including dementia and Alzheimer's care, ensuring safety and comfort Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information securely and accurately Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice-related tasks when necessary Prepare nutritious meals aligned with cultural preferences and dietary restrictions, ensuring proper food safety practices Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment for clients Follow HIPAA regulations to protect patient confidentiality at all times Collaborate with healthcare teams to ensure comprehensive patient care plans are implemented effectively Skills Proven experience in home health, assisted living, nursing homes or long-term care environments Strong knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities Skilled in assisting with ADLs, medication administration, vital signs monitoring, and dementia or Alzheimer's care Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Ability to work independently while maintaining excellent communication with clients and team members CPR certification and training in hospice or palliative care preferred Experience working with individuals from South Asian backgrounds or familiarity with South Asian cultural practices is highly valued Compassionate attitude combined with patience, adaptability, and cultural sensitivity Join us to provide heartfelt, culturally respectful care that enhances the well-being of our clients.
