In this rewarding role, you will provide compassionate, personalized care and support to residents in various settings, including group homes, assisted living facilities, and community-based programs. Duties Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling techniques like Hoyer lifts when necessary Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals Support medication administration following HIPAA guidelines to ensure privacy and safety Prepare nutritious meals and assist with meal planning while maintaining cleanliness in the kitchen area Implement and follow individualized care plans tailored to each resident's needs, including dementia care, Alzheimer's care, autism support, and neurodevelopmental disorders Maintain a safe environment by performing cleaning duties, ensuring proper hygiene standards are met in living spaces and common areas Document observations accurately in care records and participate in team meetings to update on resident progress or concerns Skills Proven experience working with individuals with developmental disabilities or in senior care settings such as nursing homes or assisted living facilities Strong patient observation skills to identify changes in behavior or health status promptly Knowledge of patient monitoring techniques including vital signs measurement and dementia or memory care practices Ability to perform physical tasks such as heavy lifting, assisting with transfers using Hoyer lifts, and manual handling safely and effectively Familiarity with medication administration procedures and adherence to HIPAA regulations for confidentiality Excellent caregiving skills combined with compassion, patience, and active listening abilities Basic first aid and CPR certification to respond confidently during emergencies Experience supporting individuals with autism or neurodevelopmental disorders is highly valued Ability to work independently or as part of a team in home health or community care environments Join us to make a positive impact every day!
