About Sycamore Health Sycamore Health is a private care coordination organization established to provide thoughtful, high-quality support to two beloved family members aging in place in Ann Arbor. The family has invested significantly in building a professional, stable, well-supported care team and takes the wellbeing of both patients and caregivers seriously. The Role We are hiring for a full-time, dedicated caregiver whose sole focus is one patient — not a rotating shared position. Depending on your certification and experience, this role can be filled at the Companion Care Aide level or the Home Health Aide level, with responsibilities scaled accordingly. The patient is warm, sociable, and benefits enormously from consistent presence, structured engagement, and calm, patient companionship throughout her day. At the Home Health Aide level, the role also includes hands-on personal care, health monitoring, and medication assistance under clinical oversight. We need someone who genuinely enjoys spending time with older adults, who brings creativity and patience to daily engagement, and who can build a trusting relationship with the patient over time. You will work as part of a structured care team that includes a Home Care Operations Manager and a supervising clinical team. Clear communication, reliability, and a genuine commitment to patient dignity and wellbeing are essential in this role. Primary Responsibilities Companionship and Daily Engagement Provide attentive one-on-one companionship throughout the day, including conversation, shared activities, music, walking, and any pastimes the patient enjoys Engage the patient in structured daily routines that provide stability and continuity Provide calm, reassuring presence during moments of agitation or confusion, using redirection and familiar orienting strategies Accompany the patient on outings, errands, and appointments as needed Maintain a tidy, safe, and comfortable environment in the patient's immediate living areas Communicate daily observations about mood, behavior, appetite, and any changes to the Home Care Operations Manager and, when relevant, the clinical team Participate in brief care coordination check-ins as scheduled Personal Care and ADLs Support the patient with activities of daily living as needed, including prompting for meals, hydration, hygiene, and dressing, while preserving her dignity and independence Home Health Aide level: assist directly with bathing, dressing, grooming, toileting, and mobility; support safe transfers and ambulation using proper body mechanics and any assistive devices in use; perform light housekeeping tasks directly related to patient health and safety (laundry, dishes, maintaining clean patient living areas) Health Monitoring and Clinical Support Monitor and document vital signs (blood pressure, pulse, temperature, oxygen saturation, weight) on the schedule set by the clinical team Observe and report changes in condition, behavior, appetite, or mobility Assist with medication administration under written nurse delegation per Michigan's nurse delegation framework; follow all MAR documentation requirements Support prescribed therapy routines, exercises, or medical equipment protocols as directed by the clinical team Assist with wound care, skin checks, and other delegated clinical tasks within the aide's verified scope of competency Documentation and Communication Complete accurate, timely care logs (and MARs, at the HHA level) each shift Communicate any changes in status or care concerns promptly and clearly Participate in care team check-ins and remain receptive to ongoing guidance from the clinical team

Qualifications Required:

Minimum 2 years of direct experience caring for older adults with complex care needs, in a home, assisted living, or similar setting Demonstrated comfort with common age-related behavioral changes, including repetition, confusion, and shifts in mood or personality Reliable and punctual; able to maintain a consistent schedule, essential for the patient's sense of safety and routine Strong interpersonal skills; warmth, patience, and genuine enjoyment of spending time with older adults Ability to communicate clearly and professionally with family members and care managers Valid driver's license and reliable transportation Clean background check and verifiable references Ability to travel up to 50% of the time (potential traveling with the family, planned in advance)

Preferred:
