Job Description

Job Summary Join our compassionate healthcare team as a Home Health Aide, where you will play a vital role in providing personalized, high-quality care to individuals in their homes. This energetic position offers the opportunity to make a meaningful difference by assisting clients with daily activities, ensuring their safety, and supporting their overall well-being. You will work closely with patients, families, and healthcare professionals to deliver exceptional home health services that promote independence and comfort. Responsibilities Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting to promote dignity and independence. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within electronic health record (EHR) systems. Provide medication administration support following medical guidelines and HIPAA regulations to ensure patient safety. Support patients with mobility needs using equipment like Hoyer lifts and assist with transfers to prevent falls or injuries. Perform household tasks including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with infection control standards. Assist with specialized care needs such as catheter care, Alzheimer's and dementia care, hospice support, and working with individuals with developmental disabilities. Document all patient interactions accurately in medical records while maintaining confidentiality and compliance with HIPAA standards. Collaborate effectively with nursing staff and other healthcare providers to coordinate comprehensive home health services. Experience Prior experience in home health care or assisted living environments preferred. Demonstrated ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Experience using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation. Knowledge of medical terminology, vital signs monitoring, medication administration, and basic nursing procedures. Long-term care experience in nursing homes or hospice settings is highly desirable. Certification in CPR is required; additional certifications such as hospice care training or dementia care are a plus. Proven ability to handle sensitive situations with compassion while adhering to HIPAA regulations and patient privacy standards.
