In this vital role, you will assist clients with daily activities, monitor their health status, and support their well-being in a respectful and professional manner. Your energy, attention to detail, and genuine care will make a meaningful difference in the lives of those you serve. This position offers an opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or palliative care. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility transfers using proper patient transfer methods and equipment like Hoyer lifts when necessary Monitor and document vital signs including blood pressure, pulse, respiration, and temperature accurately using EMR (Electronic Medical Records) systems Support medication administration following home healthcare regulations and ensuring proper documentation of medical records while maintaining patient confidentiality in compliance with HIPAA standards Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral support strategies Perform catheter care, wound dressing changes, and other medical procedures as trained and authorized by healthcare professionals Assist with meal preparation, light housekeeping tasks such as cleaning and laundry to maintain a safe and comfortable environment Communicate effectively with clients and their families about care plans, progress notes, and any changes in health status Skills Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice settings; assisted living or nursing home experience highly preferred Strong patient observation skills to detect changes in condition or behavior promptly Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of medical terminology, medication administration protocols, and basic life support (BLS) including CPR and first aid certification Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts or gait belts Excellent communication skills for client interaction and team collaboration; experience working with diverse populations is a plus Understanding of home health care assistance regulations and best practices for maintaining patient confidentiality under HIPAA guidelines Demonstrated caregiving ability with patience, compassion, and professionalism in home & community care settings Join our team to make a positive impact through dedicated patient care!
