We're looking for a Social Worker who shares this vision! This position will cover our Northwest territory. Those cities include Brighton/Commerce/Farmington/Farmington Hills/Highland/Howell/NW Livonia/Milford/New Hudson/Northville/Novi/Salem/South Lyon/W. Bloom/Walled Lk/White Lk/Whitmore Lk/Wolverine Lake/Pinckney
YOUR OPPORTUNITIES
… Completes admissions procedures including eligibility, referral procedures, intake, initial assessment and special admissions Participates in developing the plan of treatment for each patient/family, both as a consultant and also as a team member actively involved in providing direct services Assists members of the Interdisciplinary Group (IDG) to understand significant social and emotional factors related to patient care Assesses financial problems aggravated by stress or illness, and refers for financial assistance, as needed Visits patient/family to provide counseling and support Provides crisis intervention as needed Assists patient/families to develop advance directives Assists families as needed with funeral planning Utilizes existing community resources to assist patient/family Serves as a resource for members of the Hospice team about services available in the community for meeting the psychosocial/economic needs of the patient and acts as a liaison person between social agencies and the Hospice team when requested Maintains current documentation in patient's records of planning, consultation and services
WHAT YOU BRING
… Graduate of an accredited institution, MSW or equivalent education/training required in social work and bereavement services Current licensure in the State of Michigan, LMSW or LLMSW Previous hospice experience preferred

$62,400.00 per year
