Job Description:
Conduct psychosocial assessments of patients and families Develop and implement individualized care plans Provide counseling, crisis intervention, and emotional support Coordinate community resources and support services Assist with Medicaid, disability, and other assistance program applications Participate in interdisciplinary care team meetings Maintain accurate and timely documentation Ensure compliance with Medicare, Medicaid, and agency guidelines
Requirement:
Bachelor's or Master's degree in Social Work (MSW preferred) Current state Social Worker license/certification 1-2 years of social work experience (Home Health, Hospice, or Healthcare preferred) Knowledge of community resources and healthcare services Valid driver's license and reliable transportation Strong communication, counseling, and documentation skills
Preferred Qualifications:
