Job Description As a Direct Support Professional at Affinity Care MN, you will play a vital role in supporting individuals with disabilities to achieve their personal goals and live as independently as possible. You will assist with daily living activities, provide companionship, and ensure a safe and supportive environment. This is a rewarding opportunity for compassionate and dedicated individuals who want to make a positive impact in their community. Responsibilities Provide direct care and support to individuals with disabilities, including assistance with personal care, meal preparation, and medication administration. Support individuals in community activities, social events, and recreational outings. Encourage and facilitate skill development and independence in areas such as personal hygiene, communication, and daily living skills. Implement and follow individual care plans, behavior management plans, and daily routines. Maintain accurate and timely documentation of services provided, including daily logs, progress notes, and incident reports. Ensure a safe and clean environment for individuals, adhering to all health and safety protocols. Foster positive relationships with individuals, families, and other team members. Attend training sessions, team meetings, and other professional development opportunities as required. High school diploma or GED required; additional education in social services, psychology, or a related field is a plus. Previous experience as a DSP, caregiver, or in a similar role is preferred but not required. Training will be provided. Must have a valid driver's license and reliable transportation. Ability to pass a background check. Strong communication, interpersonal, and problem-solving skills. Compassionate, patient, and dedicated to supporting individuals with disabilities.
