We support individuals with disabilities in their own homes and communities, helping them live independently and reach their personal goals. We're looking for compassionate, reliable Direct Support Professionals (DSPs) to join our team. Job Summary As a Direct Support Professional, you'll provide person-centered support to individuals in their own home or family home and in the community, following each person's Individual Support Plan. Your day-to-day duties will vary depending on the person you're supporting, and may include hands-on assistance, supervision, and active coaching to help build new skills — see below for the full range of what the role can involve. What You'll Do Depending on the person's individual support plan, your day-to-day responsibilities may include: Community Participation Supporting safe transportation and mobility in the community Helping the person access and use community resources and activities Building community safety awareness Supporting social connections and communication skills Planning and joining in leisure, recreation, and social activities Health, Safety & Wellness Helping coordinate healthcare appointments and related activities Providing cueing, guidance, and support with self-care routines Supporting the person's individual health and safety plan Helping build healthy habits and self-management skills Household Support Assisting with household routines, organization, and light chores Supporting budgeting, money management, and shopping Helping with meal planning, cooking, and nutrition Supporting overall household safety awareness Adaptive & Behavioral Skills Implementing positive support strategies exactly as written in the person's plan Supporting problem-solving and self-sufficiency Helping with emotional and behavioral regulation Actively teaching and reinforcing new skills tied to the person's goals, when the person's support plan calls for skill-building General Responsibilities Following each person's Individual Support Plan and Individual Abuse Prevention Plan Documenting services provided each shift Clocking in/out of every visit using our Electronic Visit Verification (EVV) system Identifying and reporting any safety concerns or incidents per Minnesota reporting requirements Completing required orientation and training within Minnesota

timelines Maintaining client privacy and confidentiality at all times If Driving Is Part of Your Role Providing safe transportation to appointments and community activities Following all Minnesota traffic and child passenger safety laws, as applicable If Medication Support Is Part of Your Role Assisting with or administering medication exactly as directed, after completing required training What We're Looking For Must be at least 18 years old High school diploma or GED preferred; equivalent experience considered Valid driver's license, reliable vehicle, and current auto insurance (for positions involving transportation) Ability to pass a Minnesota DHS background study (NETStudy 2.0) Reliable, patient, and comfortable working independently in a home setting Strong communication skills and good judgment Experience working with individuals with intellectual disabilities and/or Autism Spectrum Disorder (ASD)
