Job Description

REVIEW OF APPLICATIONS
JULY 13TH Assists elderly clients or clients with disabilities by providing personal cares and assistance with minimal homemaking duties. May assist professional staff with community health outreach activities or events. Personal care, light housekeeping and meal preparation. Assists with and/or helps to teach clients to perform the activities of daily living. Helps clients with prescribed exercises and treatments. Assists clients with medications. Helps clients to plan, organize, and/or carry out household tasks. Helps clients to plan, organize, and/or carry out household tasks. Promotes client's mental alertness through conversation and other assigned activities. Helps clients to maintain a safe living environment. Reports any changes in the client's mental or physical condition or family situation. Daily charting on client care, and weekly checking for client assignments. Follows Agency policies regarding infection control and data privacy. Follows the plan of care for each client as determined by nurse case manager. Assists professional staff with community health outreach activities or emergency preparedness activities
MINIMUM QUALIFICATIONS
High school diploma. Certified Nursing Assistant/Home Health Aide or Licensed Practical Nurse. (CNA/HHA must be currently licensed on the Minnesota Department of Health Nursing Assistant Registry). Valid Minnesota driver's license. One to three years' work experience in a related field.
PREFERRED QUALIFICATIONS
Prior experience in Home Care or Long-Term Care Facility. Bilingual (English/Spanish or English/Somali).
HOURS:
Variable. Typical work hours 20-30 hours per week. Monday•Friday from 8:00 a.m.•3:30 pm
