Job Summary:
As a Direct Support Professional, you will provide non-medical care, including supervision and assistance provided in the home of the person or in the community to help the person receiving services achieve increased independence, productivity, and inclusion in the community. The DSP will develop real, meaningful relationships with the person supported and their support team. Staff will follow through on all duties as assigned by the primary contact, including outings into the community, assistance with ADLs, and overall supervision.
Essential Job Duties:
Assist individuals with daily living activities such as cooking/meal preparation, showering and personal cares Support the person in becoming more independent and involved in their community through frequent activity outings Assist with walks and supervision while at home and in the community Complete detailed, accurate documentation as assigned Comply with Electronic Visit Verification documentation Maintain a clean and safe living environment Implement individualized care plans and ensure individual's rights are respected Provide companionship and engage individuals in recreational activities Any other duties as assigned by the individual's primary contact within the scope of service.
Requirements Experience/Qualifications:
Minimum of a high school degree or GED (or relevant/equal education) Reliable transportation and valid driver's license Minnesota DHS background study clearance Have a Wi-Fi-enabled device for work related purposes Basic computer skills 18 years of age or older Ability to read, write, and speak the English language
