Job Description
Job Overview Join our compassionate team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of individuals requiring assistance with daily activities and health management. In this vital role, you will provide personalized in-home care, ensuring safety, comfort, and dignity for your clients. Your energetic approach and attention to detail will help promote independence and enhance quality of life for those you serve. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patient condition through observation of vital signs and overall well-being, documenting changes accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following proper protocols while maintaining compliance with HIPAA regulations to ensure confidentiality Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment within the home Assist with medical procedures such as catheter care and hospice support as directed by healthcare professionals Maintain detailed medical documentation and records to track patient progress and communicate effectively with healthcare teams Qualifications Proven experience in home health care or long-term care settings; nursing home experience is highly valued Strong knowledge of medical terminology, patient observation techniques, and infection control practices Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as first aid are a plus Ability to work independently while managing multiple clients in various home environments Experience working with individuals with developmental disabilities or disabilities is preferred Familiarity with EMR/EHR systems for documentation purposes Compassionate attitude with excellent communication skills and a commitment to patient-centered care Valid driver's license and reliable transportation are often necessary for travel between client homes Embark on a rewarding career that empowers you to provide essential care while developing meaningful relationships.
