Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs and reporting changes to healthcare providers Administer medications and manage medical documentation in compliance with HIPAA regulations Support patients with memory care needs such as Alzheimer's disease and dementia through specialized techniques and compassionate engagement Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and meal preparation Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Experience Prior experience in home health care or assisted living environments preferred Experience working with individuals with developmental disabilities or dementia care is highly valued Knowledge of medical terminology, medication administration, and vital signs monitoring is essential Familiarity with hospice care protocols and long-term care settings enhances your ability to provide comprehensive support Certification in CPR and first aid required; additional certifications such as hospice or Alzheimer's care are advantageous Proven ability to work independently while following detailed care plans and medical instructions Strong communication skills to collaborate effectively with healthcare teams, patients, and families Join us in delivering exceptional home health services that prioritize dignity, independence, and comfort.
