• Assist individuals in participating in Day Habilitation activities that support personal goals, interests, independence, and community involvement.
• Support individuals in developing, maintaining, and improving daily living, self-help, communication, socialization, safety, and adaptive skills.
• Implement Individual Support Plans, habilitation goals, behavior support strategies, and other service plans as directed.
• Provide respectful assistance with personal care needs, including hygiene, toileting, dressing, grooming, eating, mobility, and transfers as needed.
• Promote positive social interaction, choice-making, self-advocacy, and participation in meaningful recreational, educational, volunteer, work-readiness, and community-based activities.
• Ensure the health, safety, supervision, and well-being of individuals at all times while in the program, during transportation, and while participating in community activities.
• Assist with meals, snacks, dietary needs, adaptive equipment, and safe mealtime practices according to individual plans.
• Administer or assist with medications.
• Complete accurate and timely documentation, including daily activity notes, goal progress, attendance, incident reports, data collection, and other required records.
• Maintain confidentiality of individual records and personal information in accordance with agency policy, HIPAA, and applicable regulations.
• Participate in staff meetings, trainings, drills, quality assurance activities, and team planning as required.
• Demonstrate professional conduct, teamwork, reliability, respectful communication, and commitment to the agency's mission and values.
• Perform other related duties as assigned by the supervisor or program leadership. Required Qualifications
• Must be at least 18 years of age.
• High school diploma or equivalent preferred or required based on agency policy.
• Ability to pass required background checks, screenings, and registry checks.
• Valid driver's license, acceptable driving record, and ability to transport individuals if required by the position.
• Ability to complete all required orientation, training, certifications, and continuing education within required timeframes. CPR, First Aide and Med Aide Certification provided upon hire. Knowledge, Skills, and Abilities

• Basic computer or electronic documentation skills.

• Prior experience supporting individuals with intellectual or developmental disabilities is preferred but not required.
