You will provide compassionate care, support daily living activities, and promote independence while ensuring a safe and nurturing environment. This position requires a dedicated individual who is passionate about making a difference in the lives of those they serve. Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation. Provide patient care and support in various settings including group homes, assisted living facilities, and during home health visits. Monitor and document patient vital signs and health status to ensure proper care plans are followed. Engage clients in social activities to promote socialization and community involvement. Administer medications as prescribed and maintain accurate records in compliance with HIPAA regulations. Implement care plans tailored to individual needs, including support for Alzheimer's care, memory care, and dementia care. Assist with mobility using equipment such as Hoyer lifts when necessary. Maintain cleanliness of living areas to ensure a safe and hygienic environment for clients. Provide companionship and emotional support to enhance the well-being of clients. Skills First aid and CPR certification are preferred. Experience working in group homes or assisted living environments is highly valued. Strong patient observation skills to monitor changes in client behavior or health status. Ability to perform heavy lifting as required by client needs. Familiarity with home & community care practices and caregiving techniques. Knowledge of medication administration protocols and care plan implementation. Experience working with individuals with developmental disabilities, autism, or other special needs is essential. Understanding of senior care practices and long-term care experience is beneficial. Excellent communication skills to interact effectively with clients, families, and team members.
