Job Description

Job Summary:
We are seeking a compassionate and experienced Social Worker to join our hospice and home care team in Montana. This is a 13 week interim contract with the possibility of extension. The Social Worker will provide patient-centered care in home and community settings, focusing on comfort, dignity, and quality of life. Responsible for the implementation of standards of care for medical social work services and for providing care based on the Social Work Practice Act.
Key Responsibilities:
Conduct psychosocial assessments for patients and families/caregivers related to terminal illness and living environment Evaluate family dynamics, caregiver support, coping difficulties, and risk factors such as abuse or suicide Provide counseling, emotional support, and crisis intervention to patients and families Educate patients and families on advance directives and available community resources Coordinate referrals and act as liaison between patients, families, and community agencies Maintain accurate clinical documentation, progress notes, and social work records Collaborate with the interdisciplinary care team to develop individualized care plans Support patients and families with end-of-life planning, including funeral, legal, and financial arrangements Educate staff on cultural and psychosocial needs impacting patient care Oversee volunteer program activities, including recruitment, training, and coordination Participate in interdisciplinary meetings and quality improvement initiatives Build and maintain relationships with community organizations and external resources
Qualifications:
A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Minimum of one years' experience in health care, hospice experience preferred. Demonstrates good verbal and written communication, and organization skills. Must be licensed with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Assignment Details:
Interim contract: 13 weeks, with potential for extension Full-time hours with flexible scheduling based on patient needs Temp to perm position

M-F 8AM to 5
