Certified Home Health & Hospice Aide (CNA)
Pay:
$20.00-$28.00 per hour
Schedule:
Full-time Come Join TREK Healthcare! We're growing and hiring! We are seeking two compassionate and dependable Full-time Certified Home Health and Hospice Aides (CNA) to join our interdisciplinary Home Health & Hospice team. These positions are scheduled for 32 hours per week with the opportunity to flex up to 40 hours per week. Available schedules are Wednesday-Sunday or Thursday-Monday. The Home Health & Hospice Aide (CNA) is a paraprofessional member of the Interdisciplinary Group (IDG) who works under the supervision of a Registered Nurse to provide personal care services that promote comfort, dignity, and quality of life for hospice patients. This role involves direct patient care in the home setting, observation and reporting of patient status, and documentation of care provided in accordance with CMS Hospice Conditions of Participation and ACHC standards. Key Responsibilities Provide personal care services including bathing, grooming, oral hygiene, dressing, and linen changes Assist with ambulation, transfers, positioning, and range-of-motion exercises as directed Assist with feeding, meal preparation, and light housekeeping tasks as assigned Offer and assist with bedpans and urinals Provide respite support for patients and caregivers when appropriate Observe and report changes in patient condition to the RN or Case Manager Monitor skin integrity and provide care to help prevent breakdown Follow the hospice plan of care and RN assignments to ensure continuity and safety Communicate respectfully and compassionately with patients, families, and caregivers Maintain patient confidentiality and ethical standards at all times Participate in quality improvement activities as requested Qualifications Required Minimum 18 years of age CNA Licensure Ability to read, follow written instructions, and document care accurately Comfortable providing care to terminally ill patients and their families Ability to work independently in patient homes Valid driver's license and reliable, insured vehicle Current CPR certification Preferred Hospice, home health, or long-term care experience Familiarity with end-of-life care and hospice philosophy Candidates located in or near the Bitterroot Valley What We Offer!
