This vital role involves assisting clients with daily activities, ensuring their safety, and promoting independence within the comfort of their homes. The ideal candidate will bring energy, empathy, and a proactive approach to delivering personalized care that enhances quality of life. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or walkers Monitor patient health by observing vital signs and recognizing changes in condition; accurately document medical information using Electronic Health Record (EHR) systems Support medication administration following prescribed protocols and ensure proper medication management Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care strategies and behavioral support Perform catheter care, wound care, and hospice-related tasks as directed by healthcare professionals Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations Collaborate with healthcare teams to ensure comprehensive patient care and maintain detailed medical records Skills Strong patient observation skills to identify changes in health status promptly Experience working in assisted living facilities, nursing homes, or home health environments Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of medical terminology relevant to home health care settings Certification in CPR and first aid to respond effectively during emergencies Ability to perform catheter care, medication administration, and vital signs measurement accurately Experience caring for individuals with developmental disabilities, dementia, Alzheimer's disease, or hospice needs Proficiency in using mobility aids such as Hoyer lifts and performing transfers safely Compassionate caregiving skills combined with excellent communication abilities Ability to work independently while following detailed care plans and protocols Join us if you're passionate about providing compassionate home health care that truly makes a difference!
