Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation in electronic health record (EHR) systems. Administer medications as prescribed, ensuring compliance with home healthcare regulations and medication protocols. Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life support tailored to individual needs. Operate patient transfer methods safely using equipment like Hoyer lifts and other assistive devices while adhering to proper patient transfer techniques. Support clients with meal preparation, light housekeeping, laundry, and maintaining a clean living environment. Document all patient interactions accurately while maintaining strict confidentiality in accordance with HIPAA guidelines. Communicate effectively with clients, family members, and healthcare teams to ensure comprehensive care delivery. Requirements Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings. Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) including CPR certification. Familiarity with EMR/EHR systems for medical documentation and client record management. Experience working with individuals with disabilities or developmental challenges is highly desirable. Ability to perform heavy lifting safely and operate medical equipment such as Hoyer lifts or other mobility aids. Strong understanding of home health care regulations, patient confidentiality standards (HIPAA), and safety protocols. Excellent communication skills coupled with compassionate caregiving abilities for geriatric patients or those requiring hospice or dementia care. Valid certification in first aid and CPR; additional certifications related to hospice or dementia care are advantageous.
