In this vital role, you will work closely with residents to promote independence, ensure safety, and enhance their overall well-being. Responsibilities Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary Monitor residents' health by taking vital signs, observing for changes, and reporting concerns promptly to healthcare professionals Support medication administration following proper protocols and HIPAA regulations to ensure safety and compliance Prepare nutritious meals tailored to individual dietary needs and assist residents during meal times to promote independence and social engagement Maintain a clean, safe, and welcoming environment by performing routine cleaning tasks and adhering to infection control standards Implement care plans developed by healthcare teams, ensuring personalized support for each resident's unique needs Engage residents in social activities and community outings to foster socialization and mental stimulation Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), and working with individuals with developmental disabilities or neurodevelopmental disorders Ability to perform manual handling tasks safely, including heavy lifting and the use of assistive devices such as Hoyer lifts Certification in CPR and First Aid is required; additional certifications such as HIPAA training or medication administration are advantageous Strong communication skills with the ability to work collaboratively within multidisciplinary teams including social workers and healthcare providers Demonstrated patience, empathy, and a positive attitude when supporting individuals with disabilities or aging-related needs Flexibility to work various shifts including evenings, weekends, or overnight hours as needed Join us in making a real difference!
