Job Description

Overview Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. Responsibilities Provide attentive patient observation to monitor health status and report any changes promptly Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Medication reminders Support patients with specialized care needs such as Alzheimer's, dementia, memory care, hospice, and long-term care scenarios Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure precise documentation of patient interactions and vital signs. Use assistive devices like Hoyer lifts safely to aid mobility and prevent injury Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment Ensure compliance with HIPAA regulations to protect patient confidentiality at all times Collaborate with healthcare teams to coordinate home & community care plans tailored to individual needs Qualifications Prior experience in home health, assisted living, nursing homes, or long-term care environments preferred Strong knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities Certification in CPR is required; dementia training are a plus Ability to work independently while following detailed care plans and medical instructions Experience working with individuals with developmental disabilities or complex health conditions like Alzheimer's or dementia is highly valued Familiarity with EMR/EHR systems for accurate documentation of patient information Excellent communication skills coupled with compassion and patience for diverse patient needs Valid driver's license and reliable transportation preferred for live-in or community-based roles Join us today in delivering exceptional home health care that transforms lives!
