Job Description
Valley County Health System is excited to offer an excellent opportunity for a full-time Social Worker to join our dedicated Home Health and Hospice team. This position offers a 36-40 hour workweek and the opportunity to make a meaningful difference in the lives of our patients and their families.
JOB SUMMARY
Responsibilities of the Home Health and Hospice Social Worker include providing direct casework and counseling services to patients and their families as directed by the established plan of care and in accordance with the philosophy, goals and objectives of Valley County Health System.REQUIREMENTS
- Current Social Work licensure for the state of Nebraska
- A Bachelor's degree in Social Work
- A minimum of one-year experience in a healthcare setting is preferred
- Ability to provide safe and appropriate care within the standards of practice
- Independent problem-solving skills and self-directed work experience
- Experience with computers
- Demonstrated excellent written and verbal communication skills
- Demonstrated leadership skills.
