Home Care Provider (Residential with Community Services) Penacook, NH 03303 Contract Contract Home Care Provider (Live-In)
Opportunity Location:
Penacook, NH Overview We are seeking a dedicated, compassionate, and professional male Home Care Provider located in Penacook, NH or surrounding areas to welcome an individual with intellectual and developmental disabilities into their home and provide full-time, person-centered support. This is a live-in opportunity where the individual will reside in the provider's home. A single-family home is preferred. Providers must have a private, furnished bedroom available that meets Life Safety standards and passes a Fire Department inspection. As a Home Care Provider, you will create a safe, nurturing, and family-style environment that promotes independence, personal growth, dignity, and community inclusion while ensuring the individual's health, safety, and overall well-being. The ideal provider will be patient, reliable, respectful, and committed to supporting the individual in daily living activities, personal goals, and community participation. Job Duties and Responsibilities Provide support with activities of daily living, including bathing, dressing, grooming, toileting, feeding, medication administration, and range-of-motion exercises at home and in the community. Assist individuals in managing household responsibilities and daily activities in accordance with their Individual Service Plan (ISP). Schedule and coordinate medical appointments, personal activities, and maintain an organized calendar. Arrange and provide transportation for medical appointments, shopping, recreational activities, visits with family and friends, religious services, and community events. Assist with meal planning, grocery shopping, meal preparation, and specialized dietary needs. Administer medications as prescribed and maintain accurate medication records. Respond promptly and appropriately to requests for assistance and support. Teach and reinforce social, communication, and independent living skills. Encourage independence by supporting individuals to perform tasks as autonomously as possible and maximizing participation in daily activities. Maintain accurate and timely documentation of services provided. Monitor and report any changes in an individual's health, behavior, or support needs to program management immediately. Collect and record required health data, including weights and other requested information. Complete all documentation, incident reports, and required records within established timelines. Participate in meetings involving individuals, families, interdisciplinary teams, and agency staff. Attend all required trainings and maintain current certifications.
Home Care Provider Requirements:
Home Environment Residence must be located in or near Penacook, NH. Private, furnished bedroom available within the provider's home. Home must meet Life Safety standards and successfully pass a Fire Department inspection. Household must consist of adults only; no minor children (under 18 years of age) may reside in the home. Screenings & Compliance Successful completion of a criminal background check. Compliance with all home inspection requirements. Ability to meet all agency and state regulations for Home Care Providers. Transportation Reliable vehicle, valid driver's license, and active auto insurance. Ability to provide transportation for appointments, community activities, and daily living needs. Required Certifications & Training Fluent in spoken and written English. Current Medication Administration Certification (MAT). Current MOAB Level 2 (Management of Aggressive Behavior) Certification. Current CPR and First Aid Certification. Completion of Relias Training requirements. Willingness to complete any additional state-required trainings for Home Care Providers. Preferred Skills & Experience Experience supporting individuals with intellectual and developmental disabilities. Knowledge of positive behavior support, person-centered planning, and trauma-informed care practices. Strong communication, organizational, and interpersonal skills. Ability to support individuals in making informed choices and increasing their independence. Patient, dependable, compassionate, and able to maintain professional boundaries while providing high-quality care. Why Join Us?
